How Pennsylvania Medicaid home care works
An overview of Medicaid-funded home-care services in Pennsylvania, including Community HealthChoices and participant-directed services. Educational information only.
Medicaid may fund certain home-care services for eligible older adults and people with disabilities in Pennsylvania. In many counties, these services are administered through Community HealthChoices (CHC), a managed-care program for long-term services and supports.
Key concepts
- Community HealthChoices — Pennsylvania's managed program for Medicaid long-term services and supports in many counties.
- Participant-directed services — lets an eligible participant choose and direct their own caregivers, sometimes including a family member.
- Eligibility — depends on financial and functional assessments by the responsible program.
Important: not everyone over a certain age qualifies, and no family relationship automatically guarantees payment. Eligibility, approved hours, caregiver selection, and provider participation depend on individual program rules.
